Use disease, year, country, state, county, pressure, crop, and notes. Date is optional; if supplied, its year must match. Download the template.

No disease found? Leave disease blank or enter “All diseases”, with pressure set to none. This records a negative for every currently listed disease. For low, moderate, or high pressure, name the specific disease. Unknown names are skipped; imports never create diseases.
